A logo is not an answer
Health plans change their networks, and a logo on a clinic website can outlive the agreement that put it there. Canby Community Clinic does not publish a list of accepted plans for that reason. A picture of a card would look like a yes the clinic has not confirmed for you. When you call (818) 674-4414, say the name printed on your card and the kind of visit you need. Staff at 7601 Canby Ave #6B in Reseda can tell you whether that changes anything for an appointment. Bring the card if you are told to bring it. Do not photograph the card into an email or into a form on this website. Member numbers and dates of birth do not belong in a normal message. A website request is not a confirmed visit, and it is not proof that a plan will pay.
Say the plan name
Medi-Cal is not one single card with one single set of rules. Many people have Medi-Cal through a managed care plan, and the plan name is what staff need to hear. Read the name slowly, and say whether the visit is a checkup, a follow-up, or a question about a medicine. If a family member holds the card, you can still call, but the person who will be seen may need to be on the line. Ask what identification to bring. Ask whether we can see you if the plan does not cover it, or if the coverage is still pending. The clinic will not guess your coverage on this page. Medi-Cal is not required
People without any coverage call this clinic too. People whose card still leaves a visit hard to afford call too. Insurance does not, by itself, decide whether the clinic can help. A clinician here can still talk about blood pressure, diabetes risk, medicines, and the next test. That conversation is not a decision about your eligibility. Confirm any coverage question with the plan or the county, not with a blog post.
Outside tests happen elsewhere
Even when a visit at this clinic is possible, a laboratory or an imaging center may happen at another site. A plan that pays for the office visit may still treat an outside test differently. Ask where the blood draw or the scan happens, whether that site is a place your coverage uses, before you agree. Ask who calls you with the result and how long that usually takes. Do not assume a paper you signed at a lab was explained if you did not understand it. You can ask for the explanation again in English or in Spanish.
